A reminder that every player must fill out a waiver form for the fall season. Players who fail to do so will not be covered by insurance and will be considered ineligible players. Waiver forms are good for one year and expire every October. All previously signed waiver forms are now void.

Players may provide waiver forms to their referee prior to their first game. Team captains are strongly encouraged to print extra waiver forms for the player on your team who will invariably forget to bring theirs! Your cooperation with the waiver forms is expected and appreciated.
