A reminder that unless you filled out an insurance waiver form in the Fall you must fill out an insurance waiver form for the Winter! Team captains are strongly encouraged to print out some forms and bring to their games this weekend to account for the players on your team who will inevitably forget theirs at home! Again, if you filled a form out in the Fall you do not require a form but if you did not play fall then a new form is required. Forms can be handed to your head referee at your first game!
