A reminder that all players who did not fill out an insurance waiver during the indoor season must fill out a waiver form before playing in their first summer game. Waiver forms can be given to the referee at your first game. Players who do not fill out a waiver form are not insured and thus are ineligible. Teams fielding ineligible players will be subject to defaulted games. If you have filled out a waiver form during indoor (Fall 2015, Winter 2016, Spring 2016) there is no requirement to fill out another. Waiver forms filled out prior to the Fall 2015 indoor season are expired and thus new forms are required.
